Find a Dentist
Use this feature to find dentists anywhere in the United States and Puerto Rico who participate in your plan. Search by distance from your home, office or other location. Or, search by the dentist's name or specialty. It takes less than a minute to find the information you need. Results can be printed, e-mailed or sent to a PDF file. Click "Find a Dentist" on the left navigation bar to get started.
Key Contacts
Need to know how to reach Customer Service? Click "Key Contacts" for e-mail, phone, fax and address information.
Benefits Connection
This free feature connects you with up-to-date information about your benefits and benefit usage. Use it to find out exactly what your benefits cover for everything from exams and fillings to root canals, crowns and more. You can also track your benefits usage for the year, and see the total value of your dental benefits. Benefits Connection is in a secure area of our Web site. You need a password to access information. To register and obtain a password, click "Register Now" in the Benefits Connection Logon box. Already registered but forgot your password? Click "Forgot Password in the Benefits Connection Logon box.
FAQs
FAQs are "frequently asked questions." Our online database covers dozens of topics, including coordination of benefits, COBRA, determining treatment costs and more. Click "FAQ" on the left navigation bar. Don't see your question in the database? Click the "Contact Us" tab near the top of the screen to send your question to Customer Service. Most questions are answered by the next business day.
