Summer's drawing to a close and students will soon head back to school. If your college-age dependent is a full-time student, don't forget to complete Delta Dental's Student Documentation Verification Form.
This form is required to be filled out at the beginning of every fall school term to minimize claim processing delays.
You can download a copy of the Student Documentation Verification Form from our Web site at www.deltadentalnj.com. Click "Forms" on the navigation bar on the left.
