Your Proof of Membership

Once you've signed up and paid your annual fee, you'll receive a membership authorization and a membership packet will be mailed to you - usually within 7 to 10 days.

Refund Policy

If, for any reason you are not totally satisfied, you may notify Delta Dental Patient Direct in writing within 30 business days of the effective date of your membership, and Delta Dental Patient Direct will fully refund your money.
